Refined and ready

Create the Refined v2 from the Draft and the Grill decisions. It arrives as a proposal; the operator chooses Use this version or Discard, so nothing replaces the draft silently.

Outcome, scope, acceptance criteria, edge cases and open questions are separated. Unresolved questions stay visible instead of disappearing into prose.

The team edits the Refined v2 like the draft. Mark ready when the ticket is complete; the status is visible to everyone in the session.
